‍New platform for healthcare and government organizations tackles the patient attachment challenge at the heart of Ontario's Primary Care Action Plan while eliminating the digital friction burning out clinicians

IDENTOS Inc. has been awarded $975,645 from the Ontario government's Life Sciences Scale-Up Fund (LSSUF),supporting a broader $3 million investment in digital trust infrastructure for healthcare and government organizations.

The funding accelerates the launch of the company's Digital Trust Exchange (DTx), a secure platform built on FHIR (Fast Healthcare Interoperability Resources) that enables healthcare and government organizations to share data in real time while keeping privacy, consent, and access controls managed at the infrastructure layer. Its first major deployment supports Ontario Health Teams (OHTs) in meeting provincial Primary Care Action Plan targets — starting with the mandate to connect unattached Ontarians to primary care while removing the administrative and digital friction that is reported by primary and community care coordination to be at a breaking point.

IDENTOS was one of 11 companies awarded funding in this round of the LSSUF, which is investing over $18 million across Ontario's life sciences sector to help small and medium-sized enterprises scale operations and bring market-ready products to more patients.

The Challenge: Attaching Patients Without Burning Out Clinicians

OHTs face a demanding mandate to attach approximately 500,000 unattached patients during the 2026/27 fiscal year. The manual workload behind that goal has exposed three major barriers:

• Login fatigue. According to Canada Health Infoway's National Survey, 54%of Canadian physicians cite managing multiple usernames and passwords as a top digital barrier, while 40% report that health systems take too long to sign in.

• Decayed provider directories. Standard provincial databases contain limited, often inactive listings, leaving coordinators without areal-time source of truth on which clinics are actively accepting patients.

• Manual, fragmented workflows. Coordinating warm handoffs and tracking waitlists across siloed Electronic Medical Records (EMRs) remains time-consuming and prone to administrative gaps.

DTx creates a secure, vendor-neutral digital trust framework built on open standards — OpenID Connect, OAuth 2.0,and SMART on FHIR — and designed to align with health care standards. Key capabilities include:

• Single Sign-On and identity federation. Users log in once with their regional digital ID, federated with enterprise ID and existing organizational credentials, and launch tools directly within their existing workflows, eliminating password fatigue.

• A standardized app catalogue secured by policy-based access control (PBAC). Security, privacy, and access are governed by IDENTOS' (PBAC) PolicyArc technology, reducing the cost and time required for legal and Privacy Impact Assessment approvals.

The project extends IDENTOS's core PBAC intellectual property, giving individuals direct access to their own records while organizations share data securely. A product launch with a pilot is expected by the end of 2026. Beyond strengthening data integration and patient access within Ontario's healthcare system, the project positions IDENTOS for continued expansion.

Leveraging its LSSUF funding, IDENTOS is offering early-adopting OHTs limited-time subsidized launch packages, including a fully funded pilot, a bonus catalogue tool integration, and a co-design workshop.
