Integrated Care Action Summit 2026 | November 25–26, 2026

Event details:

November 25, 2026
November 26, 2026
Toronto, ON

IDENTOS will join Ontario Health Teams, Primary Care Networks, patient and caregiver advisors, and health and social care partners at the Integrated Care Action Summit 2026. The event brings together organizations working across the care continuum to strengthen coordination and build a more connected system of care in Ontario.

Across two days, participants will share practical models, examine opportunities to scale successful approaches, and discuss how teams can work more effectively across organizational boundaries. The focus is on turning integrated-care priorities into coordinated action at the community and system level.

IDENTOS brings experience in policy-based access control for healthcare, where organizational rules, patient consent, delegation, and fine-grained authorization need to work together across connected systems. That perspective is directly relevant as care delivery becomes more coordinated across providers, organizations, and digital services.
